Long Term Injury or Status Unknown on Athlete

In the case an athlete experiences a long term injury or the status is unknown (patient hasn't followed up, quit team, etc), the status on that patient can be changed to a new status option of Long Term Injury/Status Unknown - Close. This will remove the patient from the active injury & active status list. When the patient follows up or a status change is necessary, the status can be updated on that injury, and will add the patient back to the active injury & active status lists.
