Available Templates:

Within the form builder, we’ve created a library of standard forms you can customize to fit your specific needs—designed to help you build forms faster and more efficiently. These templates are available to all provider users.

We’ve also built pre calculated-based forms, such as PROs, symptom checklists, and surveys, that automatically perform scoring and calculations for you.

General Form Templates

Within the form builder we have created standard forms that you can further customize to suit your needs with the hope of speeding up the creation of your forms. These forms are available to provider users as a template.

We recommend testing the form on a test patient prior to using it in a live environment

Pre-Calculated Templates

Within the form builder we have created standard PRO/scoring specific forms. The values set are intended to adjust with your document if you add/remove components. These forms are available to provider users as a template, they can also be used for surveys. 

We recommend testing the form/survey on a test patient prior to using it in a live environment.

 

Pain self efficacy Questionnaire (PSEQ)

Assess confidence to complete activity at present despite pain. Total score calculated.

The Roland-Morris Low Back Pain and Disability Questionnaire 

Assess self-rated physical disability caused by low back pain. Total count selected will autoscore.

Eating Attitudes Test (EAT-26)

Screening measure to help determine disordered eating that needs attention from a professional. A score of 20 or more indicates the need for referral.

Knee Injury and Osteoarthritis Outcome Score (KOOS)

A self-reported outcome measure to assess the patient's opinion about the health, symptoms, and functionality of their knee. Each category (Pain, Symptoms, ADL, Sport/Rec and QOL) will auto-calculate the score. Traditionally a score of 100 indicates no problems and a score closer to 0 indicates extreme problems.

Foot and Ankle Ability Measure (FAAM)

A self-reported measure that assesses physical function of individuals with lower leg, foot, and ankle musculoskeletal disorders. Auto-calculated value will give you the level of function with 0% being the least function to 100% being the most function.

Oswestry Low Back Disability Questionnaire

A self-reported measure that is used to measure the patients permanent functional disability of the lower back. Auto-calculated values will give you an overall score 0-50, 0 being no disability, and 50 being complete disability.

The DASH

A Self-reported outcome tool that attempts to determine the amount of difficulty a patient has when doing an activity. Auto-calculated values will produce an overall score of 0-100 a higher score will indicate a greater level of disability and severity, whereas, a lower score indicates a lower level of disability

QuickDASH

A condensed version of the full DASH Instead of 30 items, the QuickDASH uses 11 items to measure physical function and symptoms in people with any or multiple musculoskeletal disorders of the upper limb.

The Lower Extremity Functional Scale

A self-reported measure of the patients ability to perform daily tasks. It can also be used to measure initial function, ongoing progress and outcome, as well as set goals. Auto-calculated vales will range from 0-80, the higher the score the greater the disability.

The Cumberland Ankle Instability Tool (CAIT) Questionnaire

A scale to measure the severity of functional ankle instability. Auto-calculated values will give a range from 0-30. A higher score indicates a higher stability.

Disablement in the Physically Active Scale

A patient reported scale a for a physically active individual who suffers from musculoskeletal injury while participating in sport and exercise. Auto-calculated values will give you a mental score as well as an overall score.

Par-Q (Short Version)

A self-reported method of uncovering health and lifestyle issues prior to starting an exercise program. Auto-calculated values for this form will produce conditionals- 1+/7 "Yes" responses are selected, a disclaimer is produced. If the patient responds "No" to 7/7, the patient then signs the document.

The Cannabis Use Disorder Identification Test - Revised (CUDIT-R)

A self-reported method of identifying cannabis use/disorder. Auto-calculated values for this form will give you a total score. It's standard that a score of 8 or more indicates hazardous cannabis use while a score 12 or higher indicates possible cannabis use disorder.

AUDIT Alcohol Screening Tool

A self-reported screening for unhealthy alcohol use, defined as risky or hazardous consumption or any alcohol use disorder. Auto-calculated values will produce a total score out of 40. 0 indicates no concern, 1-7 low risk, 8-12 risky, and 13+ indicates a high risk of alcohol use.

PHQ-9

Self-reported diagnostic tool to screen for the presence and severity of depression. Auto-calculated values will range from 0-27. It's standard that 0-4 equals "normal", 5-9 indicates mild depression, 10-14 indicates moderate depression, 15-19 indicates moderately severe depression, and 20+ indicates severe depression.

Columbia-Suicide Severity Rating Scale (C-SSRS)

A is a suicidal ideation and behavior rating scale. Answers to questions 4, 5, or 6 will trigger a conditional component with instructions for further care.

Daily Concussion Symptom Checklist

Symptom checklist from the SCAT-5 for daily monitoring. The total number of symptoms/22 and symptom severity score/132 will auto-calculate.

Psychological Readiness to Return to Sport Scale

Confidence-rated questionnaire. Scores between 50 and 60 suggest the patient is ready to return; below 50 suggest the patient may not be prepared to return to activity.

Adult ADHD - ASRS v.1 Symptom Checklist

Use the Adult ADHD - ASRS v1.1 Symptom Checklist to screen for symptoms consistent with adult Attention-Deficit/Hyperactivity Disorder and guide further evaluation.

Gad-7 Anxiety Screening

Use the GAD-7 Anxiety Screening to assess the severity of generalized anxiety symptoms; scores range from 0–4 (minimal), 5–9 (mild), 10–14 (moderate), to 15–21 (severe).

PSQI Sleep Screening

Use the PSQI Sleep Screening to evaluate sleep quality and disturbances over the past month, with higher scores indicating poorer sleep.

EDE-QS Eating Screen

Use the EDE-QS Eating Screen to quickly assess the presence and severity of eating disorder symptoms over the past week.

Step-By-Step Tutorial:

Choosing to Build a From from Scratch

Video Tutorial:

00:01: Hi, this is Nikki. Let's walk through the process of building a form from scratch. Throughout this tutorial, you'll notice two sections on my screen on the right side. It's displaying an example of the existing form that I wish to build.

00:14: On the left side is the form Builder workspace, where I will create the form step by step. As we go through this tutorial, I'll show you how to add Fields, customize them and organize them. So, that the form is ready for use by the end, you'll have a fully functional form built entirely within our system.

00:31: To begin, we're navigating into the form Builder here. I've select the documents tab from the left navigation, and then selected forms.

00:40: To add a new form. I'm going to select add in the upper right hand corner.

00:45: And now, because I'm building this form from a blank slate within our form Builder, I'll select to build form from scratch.

00:53: You see here? I've input a title for my form and a description selecting. Okay? To save.

01:01: Step one is going to be to add the form. Header, highlighted on the right hand side of the screen.

01:06: On the left side of the screen, you'll see this workspace where we'll create the form.

01:10: I'm going to drag and drop a Content field from the form Builder, toolbar onto the forms workspace.

01:19: I'm going to select my formatting options, and then select that, this is a header

01:25: and now in the text space, you can input the header

01:29: And now, select save.

01:32: Next on my form, you'll notice I have three text box components. Each containing patient demographic information to make this form cleaner and more organized Will Group these three Fields into a panel. Here's how to do it on the form Builder, drag and drop the panel component into the workspace where you want these fields to appear.

01:52: And now I will give my panel a title, it will give a clear description to the end user. What is housed within this panel?

01:59: Next select save.

02:03: Now that the panel is in place, we can start adding our items for the first two fields. Patient name and date of birth will use text Fields, simply drag and drop the text field into the panel for each item.

02:17: Now, the modal box will open for the text field. I will label it appropriately. Patient name in this example, this allows users to know exactly what information they should be entering.

02:29: Next, I want to make this form work for me. I'm going to select the data tab.

02:36: Some fields on your form can be set to auto-populate when the end user is completing them. This helps to reduce manual input for the user making the form faster and easier to complete. Now, also automatically can update on the patient's profile, if desired keeping their information current, without the extra effort on your part,

02:53: Pre-populated fields are typically drawn from the patient's detailed table specifically their demographics.

02:59: By using pre-populated Fields your ensuring consistency across forms and streamlining any data entry process for both the user and your team.

03:09: If you'd like for this form to auto update on approval, you will want to check the box below.

03:16: Next, I will select validation. I want to make this form required, so the end user cannot bypass it or save the document without completing this field.

03:26: Then check the box is required.

03:29: And now that my text field has been set the way I want it, I will select save to add it to the Forum.

03:36: I've replicated the steps to add in the patient's date of birth. Now it's time to add the phone number field in the form Builder, locate the form Builder tool drag and drop it into your desired location.

03:49: Here you will see. I've labeled it correctly patient, phone number and I've selected save

03:56: Next, I have a large area of semantics or texts that I need to input.

04:00: I'm going to put this again into another panel component.

04:05: Now, that our consent panel is in place, it's time to build the main content within the form. We'd already covered how to use a Content field, but this time, I'll demonstrate an alternative approach using the formatted content tool. This allows you to achieve the same results adding headers paragraphs and instructions that gives you additional formatting flexibility.

04:25: Here, you'll see that I've input the form data directly into what we call HTML, formatting, if you're not comfortable writing in HTML, that's completely, okay. You can always reach out to help at healthy roster for assistance.

04:38: HTML, gives you the full control over, formatting and structure again, using the content field will also accomplish the same thing.

04:48: since we've input HTML and perhaps, we're not comfortable with it, it's important to understand, you can always select Show preview,

04:57: You see here my content is now displayed exactly as it will appear on the Live Form. Be sure to select save to apply this to your form.

05:07: Next, I will again add a panel to organize my signature flow.

05:13: Once my panel is in place, I can now continue. Formatting here, I have signature and date within the same line to do. So I'm going to add a column.

05:24: I only need two columns so I'm simply going to select save. However, you can add additional columns and change their formatting if necessary.

05:33: Now, within my first column field, I'm going to drag and drop signature.

05:39: Give the field proper descriptors and select save.

05:44: Next, let's request the date to do. So I'm going to drag and drop date and time over into the column.

05:53: As you can see, on my form, I'm simply selecting. And suggesting the addition of date, not time. I'm going to select time to turn that feature off.

06:03: Now, I will uncheck the box to disable time.

06:07: Here you can see I've edited the title for this content, to remove time and only show date selecting save to apply.

06:16: Now that I've completed the build of my form, I'm going to select preview.

06:22: Preview mode is incredibly helpful, when building forms it allows you to see the form exactly as your users will. So that you can ensure everything is working correctly, perhaps by testing interactively.

06:33: Or that everything is making sense in the correct flow to exit preview mode, select the checkbox.

06:40: Once you're happy with the build of your form, or if you need to leave and come back, make sure that you select safe.

06:47: The next step in building your form is assigning it as a document type. So it's visible on patient profiles.

2. Form Building & Management

00:05: You will learn how to add and manage various form components to customize your forms effectively.

00:12: Click the button to open the form Builder interface where you can start customizing your form.

00:27: Next, you'll see edit.

00:31: This will open a modal box where you can change the title and description if necessary for your form.

00:39: Now import this is used when uploading a form template that has been previously, created in another account or by another provider user and they're sharing their import with you.

00:50: conversely, if you have created a form and you'd like to share the template with another provider user, you can export,

00:57: this will create a download to your web browser.

01:01: And lastly on the manage Form Tools on the top left-hand portion of the screen, you'll see preview.

01:09: This shows the view that the end user will experience allowing you to test your form and ensure it works as intended to exit preview select the checkmark.

01:20: Now, let's begin to work down the components within the form Builder.

01:24: First, you see a text box. This is a simple text box that allows the user to type the requested information. An example would be patient full name. The patient can type directly into this field. This format is best for short responses, or concise pieces of information.

01:43: Next, we see a text area.

01:46: This is a larger text box that allows the user to type a response across multiple lines. A common example would be to please detail in full. All yes. Responses the patient can then enter a longer more detailed explanation.

02:01: A number field will produce a fillable box to accept numbers example. How many concussions have you had in the past? The patient, then inputs a number

02:13: As you see here, a checkbox will produce a binary choice between a predefined option. Commonly a consent or acknowledgment for receiving or reviewing the document in question.

02:26: Next you see a select box. This allows you to add multiple checkboxes at one time. An example is commonly select all that. Apply the patient. Then can select as many or as little options as necessary.

02:41: Select produces a drop-down selection option here. You see in my example, please select the status at healthy roster University, the options for my dropdown are then predetermined freshmen sophomore, Junior and senior, this information can be customized as you're building it into the form.

03:01: A radio will produce a binary choice between two. Predefined options example. Do you have medical insurance? The patient is unable to select. Yes or no, but not both.

03:16: The phone. Number option, produces a billable box with the input mask which is our preferred phone number design already predetermined.

03:25: Date and time will produce a box that can host date time or both default will be to include both at the same time.

03:36: Currency gives you the ability to produce a billable box for currency input.

03:40: Format of currency is editable.

03:45: The formatted content is an area where you can build and customize content, most commonly using paragraph text with additional formatting options. While the content tool is recommended for most use cases, the formatted content field can be used to achieve similar results with a more traditional HTML editor View.

04:07: Signature will produce an electronic signature box for the user to sign electronically.

04:16: A hidden field is a tool. That is not seen by the user. That's completing the form.

04:20: Therefore, it's hidden from view often. This is used for survey users within healthy roster where you would like a protocol to be triggered if the survey has been completed.

04:34: A survey is used to produce a questionnaire in the format of question and answer with predetermined options.

04:42: The newest addition to our formula is content. This is the basic building block that lets you insert and customize general text-based or mixed media elements such as paragraphs, headings list, hyperlinks, images, and inline formatting without needing to write HTML manually.

05:02: Columns will produce a layout option. You can determine the quantity and the size of your columns.

05:09: you can then build within your columns,

05:14: A panel is another layout option. This produces a shadow box with an outline and has a header

05:20: This is commonly used when you're wanting to organize or group items within your form in one location.

05:27: Lastly, you'll see a well, another layout option. This produces a shadow box, but this time without an outline, and this does not have a header again, used for grouping information often.

05:41: And lastly, at the bottom of our form Builder, you will see the option to save.

05:45: It is essential any changes that you make you select save to apply them.

    1. Save - Saves the form in it's current build state
    2. Edit - allows editing of the form title and description
    3. Import - used to import a different form template
    4. Export - exports the forms build in a .json file for other users to import
    5. Preview - presents a form end view preview where you can check for form and function
  2. Form Components:

Text Field

A single-line text field element. The user will be able to type into this box the requested information

Example: Patient Full Name. Patient can input text. Best used for short answers.

Text Area

A multiple-line text field element. The user will be able to type into this box the requested information on multiple lines

Example: Please detail in full all “Yes” responses. Patient can input a detailed response up to the length of your text area.

Number

Produces a fillable box for numbers

Example: How many concussionshave you had in the past? The patient can input a number only.

Checkbox Produces a binary choice between  predefined option
Select Boxes

Allows you to add multiple checkboxes at one time

Example: Select all that apply. Patient can select all that apply

Select Produces a drop down selection option
Radio

Produces a binary choice between two predefined options

Example: Do you have medical insurance? Patient is able to select Yes or No

Phone Number Produces a fillable box with the input mask (preferred phone number design) already predetermined
Date/Time Produces a box that can host a Date, Time, or both
Currency Produces a fillable box for currency input, format of currency is editable
Formatted

Content area for semantics or meaning

Example: Often used for areas of consistent text that will disply on forms

Signature Produces ability to have an electronic signature
Hidden A tool that is not to be seen by the user completing the survey. Therefore it is 'hidden" from view
Survey Produces a questionnaire in the format of: question and answer with predetermined options
Content The basic building block that lets you insert and customize general text-based or mixed-media elements—such as paragraphs, headings, lists, hyperlinks, images, and inline formatting—in a flexible rich-text space without needing to write HTML manually.
Columns Layout option. Produces blank formattable columns
Panel Layout option. Produces a shadow box with an outline and has a header
Well Layout option. Produces a shadow box without an outline and does not have a header

 

 
 

Advanced Editor Tools

Setting a Form to Update on Approval

In Healthy Roster’s form builder, one of the available elements is the Text Field Component. This component collects user-entered information within a form. When creating or editing a Text Field Component, you can select the Data tab to choose which part of the patient’s profile should automatically populate that field (using Prepopulated Data Fields).

When a primary user opens the form, any fields tied to Prepopulated Data will autofill with information pulled directly from their Healthy Roster profile. These fields remain editable, and if the user updates their name, address, phone number, or any other prepopulated detail, you can choose whether those changes should also update their Healthy Roster profile.

00:00: Hi, this is Nikki. Let's review how to configure a form, so that it updates on approval. This feature is especially helpful, when you want patient demographics to update automatically, as part of your document collection process. Configuring the form is the first step to make sure those updates flow correctly.

00:19: When you're within the form Builder, you will drag and drop a text field to your desired location.

00:25: Update the title of this field here. I will be asking for my patients last name.

00:31: Next within this configuration select data.

00:36: He will then open the drop-down for pre-populated Fields, selecting the most appropriate option.

00:43: Once I've set that, I will now check the box to make sure that this is going to auto update on approval.

00:50: Now that the configuration of my text field is complete, I will select save.

00:56: Once you've completed the build process, it's important to select save to save all changes that have been applied to your form.

Step-By-Step Tutorial:

Step 1: Build your “If” question

  • This is the question that will trigger the conditional response.
    • Example: “Do you take daily medications?”
  • This question will determine whether the follow-up appears.

Step 2: Define the response options

  • Enter all available options for the question (e.g., Yes or No).
    • Important: When setting conditions, the condition is triggered by the exact value, not the label.
    • Example: Label = Yes, Value = yes.

Step 3: Identify your “Then” question

  • This is the question that will appear or remain hidden based on the user’s response.
  • We want it to show only if the user answers “Yes”.

Step 4: Set the condition

  • While editing the “then” question, select Conditional
  • The component will display True/False, meaning:
    • True → show the question
    • False → hide the question
  • Define the condition:
    • When: “Do you take any daily medications?”
  • Has the value: yes

Step 5: Confirm your setup

  • Select Preview to test your condition 
  • Your preview will then give you the ability to ensure your condition is set correctly.

3. Advanced Functionality tips and tricks

Form Sharing Overview: Exporting and Importing

If you’ve put time and effort into building a form in Healthy Roster and another provider needs a copy, you can now easily export your form template. The receiving provider can then import it into their system, making collaboration and sharing seamless.

Note: this process is only available to forms that have been built within the form builder within Healthy Roster

00:00: Hi, this is Nikki. If you've taken the time and effort to build a form in healthy roster and another provider needs a copy, you can easily export your form template they can then import it and use it within their own form Builder. Let's review

00:17: Exporting is rather simple navigate to the form, you wish to share now, select export.

00:25: You will now navigate to your advice and share the export via your desired route here. You see? I'm sharing an email with a co-worker and attaching the template that was downloaded when I hit export.

00:37: Now to import a form Builder, template navigate to the form Builder.

00:42: Select add.

00:45: And select start from scratch. If you're using a pre-existing form and your importing do understand that it will remove and strip all fields that you have already pre-built.

00:55: It's best to use a blank template when you're importing.

01:00: Now, give your form a desired name description and select. Okay.

01:05: From your toolbar, select import.

01:10: Up, you will take the file that was shared with you externally and grab it from your device.

01:16: The fields will now auto-populate onto your form and you can continue to customize as desired. Be sure to select save to save your form changes.

Step-By-Step Tutorial - Exporting:

  1. Begin by navigating to the Form Template you intend to share 
  1. Select Export from the toolbar
  1. From your device, share the export via your desired route

Step-By-Step Tutorial - Importing:

  1. Download the form template export to your device
  2. From the forms tab select, Add 
  1.  Select Start from scratch! 

You may import the template to an existing form if you'd like but the imported template will overwrite existing content on the form

 
  1. Provide your form with a name, description, and select OK

 

  1. Select Import from the toolbar
  1. Select the file from your device and upload 
  1. Once you've customized the form to your preferred layout and structure, select Save. Your form template is now successfully imported, and you can proceed with How to Create / Assign a Document Type 
 
 

How to Copy a Form

Once you've created a form in the form builder, you may find that you need slightly different version of the same form but don't want to start from scratch. In that instance we recommend making a copy of the existing from and editing the copy. This feature will help save time by allowing you to reuse an existing template with pre-filled fields, reducing the needs to recreate the form from scratch.

  1. Select the menu icon to the right of the form you wish to copy
     
  1. Select Copy
     
  1. The form will now show twice within your form listing. A Copy will be added to the duplicated version. You are then able to proceed to edit the duplicated version.

Testing/Viewing a Form

  • Use the preview function on the form builder to view and test function while still in a building flow

Saving Drafts and Making Edits

  • When creating a new form, you can click "save" at the bottom of the page at any time, and return to that form to make further edits.
  • Your form will save a draft in the Recent Forms section in the form builder that you can return to at any time.

Layout - Columns

  • WIDTH: Total is 12," we suggest to decide how many columns you want, then make the total widths equal 12.

Copy/Paste Functionality

  • All components can be copied and pasted into a new location select the copy (paper) icon
