Sending A Survey
Once you’ve created a survey, you'll need to distribute it either to an individual patient or to an organization. This tutorial will guide you through both methods using a web browser. Additionally, it’s crucial to know how to distribute surveys in the field, so we'll also cover how to send a survey to a patient via the mobile app.

- Search patient name in global search at the top of the page
 

- Select the menu next to the appropriate patient's name, and select view.
 

- Click on the Surveys tab in the patient profile
 

- Select "Send" to retrieve a list of surveys available for this patient.
 

- Select the appropriate survey from the drop-down menu.
 

- Select "Send" when ready to send.
 

Sending A Survey Step-By-Step Tutorial (To Entire Organization): WEB
- Select the Organizations tab on the left navigation bar.
 

- Select the organization to which you need to send the survey.
- Select the Surveys tab from the organization's navigation bar.
 

- Select the menu next to the survey you'd like to send and select Send.

Please note: You can only send an individual survey through the app.

- Log in to your Healthy Roster app, and select the student you are sending the survey to
- In the patients profile, tap on documents
 

- Tap the + sign in the top right-hand corner of the screen
 

- Select “Send Survey”
 

- From the drop-down menu, select which survey you'd like to send
 

- Double check the phone number is correct and tap Save
