Survey Summary Report Video Tutorial: Step-By-Step Tutorial

Survey Summary Report

The survey summary report is a global look at each of your surveys, including information about whether the survey has been sent or if it has been completed. You can use this tool to organize by org/team/status to get you the information you need quickly

  1. Select the document tab.
     
  1. Select Surveys
     
  1. Select the menu icon to the right of the survey you wish to review, click View Survey Responses
     
  1. Review the filters of the report. You are able to view by Organization (default), filter down to team, and show only certain status options.
     
  1. Select the column to readjust its order
     
  1. Review Action Menu Items:
     
Edit Contact Info: Opens a Modal box to edit patient details

 

Send Survey: Delivers the survey to the patient A green notification will confirm the send
Open Athlete: Navigate to patient profile Will navigate you to the patient's profile
  1. Surveys that need reviewed or that have an urgency will have a few identifiers. The patient column will be color coded per the urgency (Elevated or critical), and Requires Follow-Up will have a checkmark 
     
  1. Select the menu icon for your patient's action, and select Open to be navigated to the Survey response
