EMR - Reporting Template Guide
Table of Contents
Use this guide to quickly find the right report for your needs. Templates are the best starting point — most of what you're looking for is already built. When you need something custom, these templates are also the foundation to build from in most cases.
Clinical — Documentation and patient care records, including Problems, Encounters/Treatments, Referrals/Follow-Ups, and the background data collected day to day.
Encounter log Lists all encounters (treatments) line by line. One encounter with 3 services will appear as 3 rows — meaning a patient may show multiple times. Tip: To see one row per encounter, add an Encounter ID column and group by it. |
Problem log All documented problems/injuries in the system, regardless of whether an incident report (SOAP note) is attached. Use this when you need a full list of injuries without filtering by documentation status. |
Problem breakdown Visual dashboard of injuries by body area, condition, and graduation year. Only includes problems linked to an incident report (SOAP note). Tip: You can add background questions to enrich this report with custom data your providers are collecting. |
Allergies, medications & medical alerts A concise list of all patients flagged for allergies, medications, or medical alerts. Great for sharing a 'red flag' or alert report across your team. |
Emergency contacts All emergency contacts on file across your patient population. Tip: If an athlete is missing from this report, they have not yet added emergency contacts to their patient details. |
Referral log All referrals (follow-ups) documented in the system — things providers are sending patients out for, not self-referrals. |
Background answers by patient All background question answers for a specific patient across encounters, incident reports, referrals, and patient details. Useful for tracking a patient's progress on a specific metric over time. |
Recent background question answers The most recent background question answer per patient — a snapshot across your population rather than the full history. Good for population health management when you want a quick pulse on a specific metric. |
Event participation Participation and exposure records tied to scheduled events. Helpful for tracking athlete participation history. |
Insurance submissions History of insurance submissions to third-party integration partners and their current status. Use this to spot delayed submissions caused by missing information. Tip: This report works for all third-party insurance integrations and refreshes automatically overnight after data is resubmitted. |
Invitations Tracks user invitation status. Helpful for troubleshooting patient account signup issues and measuring engagement metrics. |
Financial — Cost and revenue reporting across encounters and referrals— useful for understanding your return on investment
Encounter cost breakdown Visual dashboard of encounter service costs by organization, team, service, and billable status. Shows your return on investment for what providers are rendering to patients. |
Referral revenue breakdown Visual dashboard of referral revenue by healthcare organization, provider, and service type. Shows how much money is going out and where — useful for understanding referral ROI. |
Engagement — System usage metrics at the organization and provider level, helping you identify who's using the platform well and where additional support or training may be needed.
Admin — Access and account management reporting, primarily used for auditing, compliance, and troubleshooting user access issues.
Organization engagement System usage broken down by organization. Helpful for identifying which organizations may not be fully utilizing the platform. Tip: Compare organizations of similar size and provider count to spot gaps in documentation. For example, if two schools have similar patient populations and provider counts, lower engagement at one may point to a need for additional support. |
Provider user engagement System usage by individual provider. See who is documenting what and spot providers who may need additional training or support. Tip: Ideally every problem (injury) should have an associated incident report. This ratio shows whether providers are opening injuries without following up with SOAP notes — and can reveal where additional education may be needed. |
Events A list of all scheduled events in the system. |
Access logs System access logs by user and patient. Primarily used for auditing and compliance purposes. |
Invitations Tracks user invitation status. Helpful for troubleshooting patient account signup issues and measuring engagement metrics. |
